We offer two ways of completing the forms:

1. You may type directly on the PDF forms and then print out the forms to mail  to the address on each form

2. If you prefer to fill out the form by hand, simply print the form, complete and mail to the address on each form.

Prepayment Reservations and Merchandise must be sent by US mail to the person indicated on each form, with payment by check included.

For Parade Registrations (Rescue and Veterans) at the Banquet, no payment is required. The completed forms and requested photographs can be sent by US Mail to Kerrie Bryan, 3044 McIntosh Drive, Longmont CO 80503.   However, if you prefer to scan the photographs and the completed forms, you may email them to Kerrie Bryan at aussee@comcast.net. 

For the Getting to Know You at the Specialty Questionnaire, no payment is required.  The completed form and requested photographs can be sent by US Mail to Kerrie Bryan, 3044 McIntosh Drive, Longmont CO 80503.   However, if you prefer  to scan the photographs and the completed forms, you may email them to Kerrie Bryan at aussee@comcast.net. 

The Premium List will be mailed by Onofrio Dog Shows to all ATCA Members in the US and Canada.  ATCA is providing Onofrio Dog Shows with mailing labels for that purpose.

The Specialty Packet in hard copy will be mailed to anyone who does not have an email address in the current Blue Members Directory.  If you wish to receive a hard copy even though you have email and internet service available to you, email your request to wismiss@comcast.net and provide your correct mailing address, please.
